• Our program sends a physical therapist or occupational therapist directly to your home for a pre-operative, home-safety evaluation.
• By identifying and mitigating potential hazards in advance, we ensure your home supports a safe and efficient recovery.

Meet Health Link: 

Serving the Bay Area for nearly 20 years, Health Link has learned to focus on you, our patients. By focusing on your recovery, we live out our mission to empower you and assist you to regain some independence. The preoperative visit is free of charge and the post-operative home health services will be covered by insurance. Your provider will send us the referral and our intake team will call you to schedule your visit before your surgery. We will schedule our after-visit appointment for 24-48 hours after surgery to begin the recovery process quickly. We look forward to the opportunity to support your health.
