- Right to Respectful and Nondiscriminatory Care: Every patient has the right to receive care that is respectful, considerate, and free from any form of discrimination based on race, color, religion, national origin, age, disability, sex, or sexual orientation.
- Right to Informed Consent: Patients have the right to receive all the information necessary to make informed decisions about their care plans. This includes information about their health status, treatment options, potential risks, and the expected outcomes, in a manner that is understandable to them.
- Right to Privacy and Confidentiality: The confidentiality of health information and the privacy of individuals receiving care are paramount. Patients have the right to expect that all communications and records pertaining to their care will be treated as confidential.
- Right to Participate in Care: Patients have the right to be involved in all aspects of their care, including the development and modification of their care plan. This includes the right to refuse treatment and to be informed of the consequences of any such refusal.
- Right to Voice Grievances: If a patient or their family has concerns or complaints about the quality of care or treatment, they have the right to voice these grievances without fear of reprisal or discrimination. Every complaint must be investigated, and a resolution should be sought in a timely manner.
Patient Responsibilities
- Participation in the Care Plan: Patients are responsible for participating in the development of their care plan and expressing any concerns or preferences regarding their treatment.
- Providing Accurate Health Information: To receive the best possible care, patients are responsible for providing accurate and complete health information to their healthcare providers, including past illnesses, hospitalizations, medications, and other matters relating to their health history.
- Compliance with the Care Plan: Patients are responsible for following the care plan agreed upon with their healthcare provider. If unable or unwilling to follow the plan, patients should communicate this to their provider to make necessary adjustments.
- Respecting Healthcare Providers: Just as patients have the right to respectful care, they also have the responsibility to treat their healthcare providers with respect and dignity.
- Communication: Patients should communicate any changes in their health or condition to their healthcare provider. They should also inform the provider if they do not understand their care plan or the instructions given to them.
